There is a significant number of people that despite having a caring responsibility do not recognise themselves as carers. Our aim is to identify these hidden carers and provide them with appropriate support to continue their caring role.
We work with referral partners from different areas to identify and work with carers from all communities in Stoke and Staffordshire.
We are particularly keen to engage with communities for whom English is not a first language, carers from rural communities, mental health carers, carers of people with learning disabilities and or autism, carers of people with substance misuse issues and end-of-life carers.
Our services are available to all carers who are resident or care for a resident in Stoke-on-Trent or Staffordshire or registered with a GP in Staffordshire or Stoke-on-Trent.
Currently we accept referrals from carers themselves, statutory agencies, relatives, friends and voluntary organisations.

You will be asked for some basic information about the carer to allow us to get in contact.
Once we get in touch with the referred carer, we will invite them to undertake an assessment as a starting point for an individual support and care plan.
