This is an initial assessment with one of our highly trained, local, triage team members. This assessment will offer any information, advice and guidance you might need. We can also signpost you to any other organisations that might be able to help your particular need. The team are on hand to talk about your caring role and no question is a silly question. The team will assess your situation and provide you wth the right level of support to help meet your needs. This call can take as little as 10 minutes but is totally based around you and what you wish to share.
If following your triage assessment the team feel you need further, intensive support, or if you specifically request a full assessment, you will be allocated a key worker known as a 'Generalist Advisor'. They will call you to arrange completing the Universal Carers' Assessment (and/or Star Assessment) with you, at a time and location that is convenient. Your key worker will remain allocated to you until you no longer need our support.

A full Carers' Assessment can take place over the phone, face to face, at either of our offices in Stafford or Stoke-on-Trent, at one of our outreach points, or in your own home. This assessment is your opportunity to talk freely and openly about how your caring role affects you, so that we can help create a plan of action of how we help you to move forward. The STAR assessment aims to track where you are now and aims to show you where you are after you have worked with us.
The assessment can be completed over the phone or face to face and will usually take around 1-2 hours to complete.
After your assessment is completed your allocated key worker/generalist advisor will create a plan that summarises all of the mutually agreed actions from the assessment.
This will be known as your Care Plan.
Your worker will support you in completing this plan and action anything tasked to them. They will also be on hand at any point, should you need them. They will complete a review with you 6 and 12 weeks later.
After your care plan tasks are completed and you are beginning to feel more supported, we will continue to keep your details on file. That way, should you ever need to access us again in the future, you will be able to do so. However, we will not cold call you or share your details with any third parties.
If you wish to still access our groups you will then be supported by our activities coach, who can help to find the right group for you.
